Incident reporting - a team effort
To produce a quality SAFEX report at any of the three levels requires a team effort. Members of the SAFEX team are the:
- SAFEX Contact in each member company who notifies us of the incident and also circulates the incident information SAFEX distributes within their organization;
- SAFEX Secretariat who processes the initial Incident Notice;
- Editorial Committee who scrutinizes any Investigation Reports SAFEX receives;
The team achieves its goal when members successfully interpret and apply the learning from an incident in their organizations.
SAFEX Contacts
They are the people who represent member companies in any SAFEX interaction. As senior people they have many demands on their time but are committed to improve the HSSE performance of their explosives operations.
Incident reporting starts and ends with the SAFEX Contact. Other SAFEX members depend on the diligence of the Contact in every member organization
The Editorial Committee
Members of the Editorial Committee are experienced investigators and reporters of incidents. The Committee member that is best placed to review an incident is nominated to scrutinize the Investigation Report for that incident before it is distributed.
The Committee’s function is to assist SAFEX in improving the quality of incident reporting by providing a fresh “pair of eyes” that can assist in identifying possible learning opportunities. By maximizing the learning members can obtain from every incident that is experienced, we can show some benefit for the pain incidents cause.
